Our Community Nursing services are delivered by qualified Registered Nurses to support participants with complex health needs in the comfort of their own home. We work closely with participants, families, GPs, specialists, allied health professionals and Support Coordinators to deliver safe, evidence-based clinical care that promotes independence, improves health outcomes and reduces unnecessary hospital admissions.
- Comprehensive nursing assessments.
- Development and review of individualised nursing care plans.
- Medication management and administration.
- Diabetes management, including blood glucose monitoring and insulin administration.
- Wound assessment, wound management and pressure injury care.
- Continence assessment and management.
- Catheter care (including indwelling, suprapubic and intermittent catheter management).
- Stoma care and complex bowel care.
- Enteral feeding support, including PEG and nasogastric (NG) tube management.
- Respiratory care, including tracheostomy care and ventilator support (where clinically appropriate).
- Intravenous (IV), subcutaneous and intramuscular medication administration, where clinically indicated.
- Chronic disease management and ongoing health monitoring.
- Post-hospital discharge nursing and transitional care.
- Palliative and end-of-life nursing support.
- Clinical observations and early identification of changes in health status.
- Health education for participants, families and carers.
- Clinical support, supervision and training for support workers delivering participant-specific health supports.
- Coordination of clinical care with GPs, hospitals, specialists and other healthcare professionals.
- Complex bowel care and stoma management.
- Subcutaneous, intramuscular, and intravenous medication administration.
