Horizon Living Group Pty Ltd is a Registered NDIS Provider supporting participants across the Perth Metropolitan Area. Our services are built around each person’s own goals rather than a fixed package, and everything below can be adjusted as those goals change over time.

Our Approach to Support

No two participants need the same thing, so we don’t treat support as one-size-fits-all. Plans are shaped around individual goals, preferences and levels of independence, and adjusted as circumstances shift. What stays constant is the standard behind it person-centred care, responsive communication, and a level of service quality shaped by our Director’s background in healthcare and aged care leadership.
