Personal Care NDIS Perth | Personalised In-Home Support
Horizon Living Group provides Personal Care services to NDIS participants across the Perth Metropolitan Area. Care that fits the person, not a fixed routine handed out the same way to everyone. As a Registered NDIS Provider, our team works alongside participants, families, support coordinators and healthcare professionals to deliver support that is safe, consistent and properly managed from the first visit onward.

Personal Care covers the daily support many participants rely on to manage hygiene, mobility and routine. Showering. Dressing. Moving safely from one room to another. For some participants, this support is occasional. For others, it forms the structure of every single day.
What matters most is how the support is delivered, not just what tasks are ticked off. Privacy, comfort and individual preference guide every interaction, and independence remains the goal throughout. Safety and personal wellbeing are treated as essential, never optional.
Support workers assist with showering, bathing and grooming, always with attention to comfort and personal choice. Dressing support accounts for mobility limitations as well as individual preference, because the two do not always line up, and both need to be respected.
Toileting assistance and continence support are handled discreetly and professionally. Where mobility needs exist, support workers assist with transfers and safe positioning around the home.
Meal preparation support is available for participants who need help with nutrition. Medication reminders are provided where appropriate. Morning and evening routines are supported so each day has a structure participants can depend on.
These supports are often combined with other services under a participant’s plan, including Household Tasks or Community Nursing.
Person-centred care. That phrase gets used across the entire sector, often loosely. At Horizon Living Group, it shapes how support is actually delivered — matching support workers to participants, maintaining that relationship over time, and adjusting the approach as circumstances change.
Privacy and dignity are maintained throughout every session, without exception.
Communication with families, support coordinators and healthcare professionals stays ongoing rather than occasional. This keeps support plans accurate, keeps everyone informed, and strengthens continuity across related services such as Community Participation, Travel & Transport and Development of Daily Living Skills.
The Director brings more than ten years of healthcare and aged care leadership experience. That background shapes governance, quality management and a working understanding of what person-centred delivery actually requires day to day, not just what it looks like on paper.
Support workers are experienced. Communication is reliable. Support planning is built around the individual rather than a generic template, and referrals are processed promptly, so participants are not left waiting for support they have already been approved to receive.
