Horizon Living Group supports NDIS participants across Perth to get out into their community, build friendships and pursue activities they actually care about. We’re a Registered NDIS Provider, and our Community Participation support is shaped around each person’s own goals rather than a fixed program. Families, Support Coordinators and healthcare professionals work with us because our communication is consistent and our support workers show up reliably, week after week. Whether someone wants help attending a local footy match, joining a class, or simply getting more comfortable in social settings, our team is there to make it happen.

Community Participation is NDIS-funded support for social, recreational and community-based activities. It might mean help getting to a weekly art class, support at a community event, or simply having someone alongside you while you try something new.
Independence often grows out of small, repeated wins. A support worker helping with transport and communication one week can mean a participant feels ready to attend on their own the next. That’s the gradual shift we aim for.
Isolation takes a toll on wellbeing, and this support exists partly to counter that giving people a practical way back into social life, or a first step into it if they haven’t had one before.
No two support plans look the same. Some participants want structured, regular outings. Others are working toward one specific goal, like joining a local sports club. We build around what’s actually wanted, not a template.
Our support workers accompany participants to social outings and local events a market, a match, a movie, whatever suits. For those working on daily independence, this can stretch to shopping trips, appointments and the everyday errands that come with living well in the community.
Hobbies matter too. Art, music, fitness, gardening groups we support participants to keep doing what they enjoy, or to try something they’ve been curious about. Confidence and social skills tend to develop naturally through this kind of regular, low-pressure interaction with others.
We also help participants connect with established community programs and clubs relevant to their interests, so support isn’t just about the outing itself but about building an ongoing sense of belonging. This often works alongside other supports we provide, including Development of Daily Living Skills, Travel & Transport and Personal Care, so everything across a participant’s plan is coordinated rather than siloed.
Person-centred care isn’t a line we use for marketing it’s how support plans actually get built. Participants choose the activities. We provide the support to make them achievable.
Independence is encouraged wherever it’s realistic, with just enough assistance to keep things safe and manageable. Respect and individual goals stay front and centre, even as circumstances change over time.
Communication with families and Support Coordinators is ongoing, not occasional.
When plans shift, we adjust. Where possible, we keep the same support workers involved with a participant long-term, since familiarity tends to make people more comfortable and more willing to engage.
