Our Referral Process
Step 1 – Initial Enquiry

Step 2 – Consultation
A consultation is arranged to discuss the participant’s goals in more detail. This may involve the participant, their family, and their Support Coordinator, depending on individual circumstances. The aim is to build a clear picture of the support required and how it fits within the participant’s broader NDIS plan.
Step 3 – Individualised Support Planning
Based on the consultation, a tailored support plan is developed. This plan reflects the participant’s specific goals, preferences and funded supports, ensuring services are aligned with what has been agreed in their NDIS plan.
Step 4 – Service Commencement
Once the support plan is finalised, services are scheduled and the participant is introduced to their support team. Clear communication continues throughout this stage, so participants and families know what to expect from day one.
