Who We Are
Most people who’ve dealt with disability support services know the frustration of forms that go nowhere and phone calls that don’t get returned. Horizon Living Group was built to work differently. We support NDIS participants, families, support coordinators and healthcare professionals across Perth, and the offer is fairly simple even if the execution takes real effort day to day professional service, honest communication, and a focus on what the individual actually wants rather than what’s convenient for us to deliver.

Our Story
There isn't a dramatic founding story to tell, and we're not going to invent one. Horizon Living Group came from a fairly plain observation: participants deserve providers who follow through on what they say, who communicate properly, and who treat someone as a person rather than a file number sitting in a queue.
Leadership & Experience
Our Director brings more than ten years of leadership experience across Australia's healthcare and aged care sectors, spanning governance, quality management and person-centred service delivery the kind of grounding that comes from years inside real healthcare systems rather than a weekend course.
Our Values
Professionalism runs through everything we do, but it's paired with respect for each participant's own choices and pace, because the two don't mean much without each other. Integrity means doing what we said we'd do even when nobody happens to be checking, and compassion sits behind every interaction rather than only the difficult ones.
Our Approach to Support
No two participants are the same, so no two support plans look the same either. We build individualised supports around each person's goals, preferences and abilities across Community Nursing, Personal Care, Household Tasks, Community Participation, Travel & Transport, and Development of Daily Living Skills. Choice stays with the participant throughout our job is to support independence and confidence, not to decide on someone's behalf what that independence should look like and we stay in close contact with families and support coordinators the whole way through, because good support rarely happens quietly behind closed doors.
